Your Annual Wellness Visit is a yearly appointment with your Primary Care Physician. This visit is used to assess your current health and risk factors as well as to create a plan to maintain your overall health and keep you out of the hospital.
An Annual Wellness Visit May Include:
A check of your height, weight, and blood pressure
Obtaining your past medical history and your family medical history
Health risk assessment
Health Education and Preventative Counseling-Falls, Nutrition and Smoking
Order your necessary screening procedures based on age: Colonoscopy, Bone Density, Diabetes, Heart Disease, Mammogram, and Weight Management.
Review and administration of necessary age-based vaccinations.
